Abstract
Although the emphases on ¡®publicness¡¯ and health care provided by public sector has been evolved in South Korea, theoretical basis of publicness of health and health care is still fragile. As policies and practices of health care based on the value of publicness are under active debates, philosophical and theoretical foundations are need to be more systematic and relevant.
As the criteria of ownership is not sufficient condition to fulfil the value of publicness, it is not clear ownership is essential requirement to achieve publicness. The single criteria of ownership, focusing individual provider or institution in particular, is not applicable in explaining the reality as well as theoretical aspects on whether publicness is achieved or not. Performing publicness of health care is related to multiple factors at various layers and dimensions, but current framework of publicness is only concentrating on individual elements without open-system thinking.
This paper discuss how we could extend the concept of publicness and ¡®public health care¡¯ to include public system, public eco-system, public regime, and health regime, as tentative but promising theoretical frameworks, more relevant to explain reality and challenge in achieving publicness in health and health care. Regional health regime is also discussed, considering geopolitics, decentralization, and ¡®production regime¡¯ of health care.
